Immunizations (TK-12)

State law requires that for unconditional admission to school, all students under 18 shall be fully immunized according to requirements of the California Department of Health Services. Immunization records are required at the time of enrollment.

California schools are required to check immunization records for all new student admissions at transitional kindergarten (TK)/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child’s Immunization Record as proof of immunization. 

Required Immunization Records

Students Enrolling in Grades TK–12

 

  • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses
    (4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday.)
    For 7th–12th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday. 

     
  • Polio (OPV or IPV) — 4 doses
    (3 doses OK if one was given on or after 4th birthday. ​Oral polio vaccine [OPV] doses given on or after April 1, 2016, do not count.)​ 

     
  • Hepatitis B — 3 doses
    (Required at admission to any grade except 7th grade​) 

     
  • Measles, Mumps, and Rubella (MMR) — 2 doses
    (Both given on or after 1st birthday) 

     
  • Varice​lla​ (Chickenpox) — 2 doses

Students Entering Grade 7

 

  • TetanusDiphth​eria​Pertussis (Tdap) —1 dose
    (Whooping cough booster usually given at 11 years and up)

     

  • Varicella (Chickenpox) — 2 doses
    (Usually given at ages 12 months and 4-6 years)​

 

Frequently Asked Questions (FAQs)

  • Students may be admitted on a “conditional” basis without all required immunizations being up to date. The student will be required to complete the immunizations according to California Department of Health Services California Immunizations Requirements for Pre-Kindergarten and K-12 to remain in school.

  • Medical exemptions to immunizations are allowed by law and must be requested by parents on the Medical Exemptions from Immunizations for School and Child Care (CAIR) website.  

    Steps: 

    1. Register on the CAIR website.
    2. Complete the parent section of the application.
    3. Request that the student’s health care provider complete their section of the application.  
    4. Obtain a printed copy of the completed application.
    5. Submit the complete application to the student’s school.
    6. The California Department of Public Health will review all requested exemptions.
